Determine Eligibility
Our enrollment specialists will review your family's eligibility for the California Title 5 Program and explain which documents apply to your situation.
Complete an Enrollment Packet
Families complete the required enrollment forms with assistance from our staff. Enrollment packets are available through the center and online. (Forever Friends ELC)
Submit Required Documentation
Families are typically asked to provide:
Parent or guardian photo ID
Birth certificates or birthing announcements for all dependents
Child's up-to-date physical and immunization records
Proof of California residency
Enrollment Review
Our team verifies all required documentation, confirms eligibility, and answers any remaining questions.
Child Begins Their Journey
Once approved, we'll schedule your child's start date, introduce your family to the classroom, and help ensure a smooth transition into our program.
